PROWES: CiRT utility in guiding treatment decisions

  • Interim results from PROWES, a Prospective Real-World Evidence Study of Oxford BioDynamics’ (OBD) EpiSwitch CiRT(Checkpoint inhibitor Response Test), have shown its clinical utility in guiding immunotherapy treatment decisions across multiple solid tumour types and patient populations. The data have been published in Cancers in a peer-reviewed paper: “Clinical Utility of the EpiSwitch CiRT Test to Guide Immunotherapy Across Solid Tumors: Interim Results from the PROWES Study”.
  • PROWES is a pan-US prospective registry study enrolling patients across multiple speciality oncology institutions. It was designed to clinically validate the ability of CiRT in guiding treatment decisions by comparing the predicted response and the actual response rates to immune checkpoint inhibitors (ICIs) across a broad range of advanced solid tumours in the real-world setting.
  • In the 205 patients enrolled, the blood based CiRT influenced treatment decisions (initiation, escalation, de-escalation, avoidance) in 61% of cases. In patients with predicted ‘low response probability’ almost half avoided likely ineffective ICI therapy (typically only 20-40% of patients respond to ICIs) sparing them from unnecessary exposure to potentially serious side-effects, whereas nearly three-quarters of patients with ‘high response probability’ continued with or escalated their ICI treatment. Additionally, CiRT results closely matched real-word patient outcomes and early data suggest a role for CiRT in monitoring emerging resistance to ICIs.
  • PROWES data will contribute to the CiRT Health Economics and Outcomes Research (HEOR) evidence package to support inclusion in clinical guidelines, a critical step in driving wider CiRT adoption. OBD will apply for inclusion in US National Comprehensive Cancer Network (NCCN) guidelines later in 2025. CiRT is currently available in the US (reimbursed under a unique CPT code) and privately in the UK (including through BUPA).

Trinity Delta view: EpiSwitch CiRT addresses a clear need in helping guide ICI therapy, accurately differentiating between non-responders and responders, thus avoiding unnecessary side-effects and costs. The further clinical validation provided by PROWES data and the expectation that Oxford BioDynamics will apply for clinical guideline inclusion during 2025 should drive wider CiRT adoption by oncologists, growing test revenues, and making CiRT more attractive to potential partners. OBD’s near-term focus remains on securing one or more partnerships, collaborations and licensing deals to maximise the revenue potential of its portfolio of innovative and clinically robust tests. Longer-term sustainable test volume growth will require substantial ongoing commercial investment, and thus third-party partners or further funds are needed to deliver on the sizeable potential market opportunities for CiRT, PSE, and the EpiSwitch test portfolio. Commercial discussions are ongoing with multiple diagnostic/pharmaceutical companies, with a recent Google Cloud collaboration designed to make the OBD EpiSwitch platform more accessible to pharma and biotech companies and create future commercialisation opportunities.
